With a background in physiotherapy, Peter has worked as a patient safety consultant delivering training programs on root cause analysis and patient safety. He was formerly the Associate Director of Patient Safety at the National Patient Safety Agency in London where he led and managed clinical teams aiming to reduce preventable patient harm.
Associate Professor Bernie Harrison:
Bernie Harrison has over 30yrs experience in health care as a clinician, researcher and quality and safety expert. She has extensive knowledge in designing and delivering quality and safety training programs in the Asia pacific region, with former roles as the Executive Director of Hospital Performance for the National Health Performan.
Part 1: Understanding Human Factors and how they can impact on serious patient safety events:

In this session, participants will explore the science of human factors within the healthcare environment and discuss their potential impact on serious patient safety events. Participants will define the principles of human factors and examine the human abilities, limitations, behaviours and processes which influence everyday interactions. Approaches to improve the safety culture within clinical practice will be explored, including how to enhance situational awareness to improve clinical decision-making and useful strategies, resources and tools to address diagnostic errors and cognitive bias.
Part 2: Learn how to investigate serious patient safety events using Root Cause Analysis:

Using a systems-thinking approach, participants will delve into the critical process of investigating serious adverse events that result in patient harm. Applying real case examples, practical insights to the steps involved in root cause analysis and the policy and legislative frameworks that govern root cause analysis will be explored. Participants will discuss how to conduct staff interviews to identify underlying sources of clinical process failure and enhance skills to formulate strong recommendations based on findings. Development and implementation of a robust plan to mitigate future risk will equip participants with the skills and resources required to navigate to a safer healthcare system.

This workshop will introduce the ANZICS Clinical Quality Registry and how data can be used to monitor patient outcomes and ICU performance.
Using a fictional case example, participants will analyse ICU benchmark data from the Adult Patient Database (APD) , Critical Care Resources (CCR) Survey and Critical Health Resource Information System (CHRIS) to investigate possible contributing factors to explain why an intensive care unit has been flagged as a potential ICU outlier.
With 30 years of experience in ICU benchmarking, the ANZICS CORE Outlier Management Program will be demystified. We will explain the Australian and New Zealand Risk of Death (ANZROD) Model and issues which may impact on data quality.
